Forms & Documentation

We understand that life sometimes requires clinical documentation. Once we've built a strong therapeutic relationship, we're happy to help with the paperwork.

Minimum 9 Sessions

You must have completed at least 9 therapy sessions with your Maunda therapist before documentation services become available. This ensures your therapist has adequate clinical basis for any documentation provided.

3-Month Relationship

A minimum of 3 months of ongoing care is required. This "get to know you" period allows your therapist to provide accurate, clinically-supported documentation on your behalf.

Why these requirements? Ethical clinical documentation must be grounded in a genuine therapeutic relationship. These minimums ensure your therapist can speak confidently and accurately about your treatment, diagnosis, and needs.

Transparent Pricing

Simple, per-page pricing for all documentation requests.

Pages 1–10

$5/page

For documents up to 10 pages in length

  • 1-page form = $5
  • 5-page form = $25
  • 10-page form = $50
Pages 11+

$10/page

For every page beyond the first 10

  • 12-page form = $70
  • 15-page form = $100
  • 20-page form = $150

Payment is due at the time of the documentation request. Turnaround is typically 5–7 business days.
